Electronically Certifying a DH680

Creating an electronically-certified DH680 (bluecard) is simple and fast - two things you need. 

Once your physician has approved who can certify a DH680 and who can create one, the local administrator on your Florida SHOTS account needs to set the user to the appropriate level of authority. This is done on the personnel maintenance screen for the user ID by selecting “Yes” from the drop-down menu for “Certify Form 680.” If the user is only granted the privilege of creating a DH680, leave “No” in the “Certify Form 680″ box but be sure to select “Yes” for “Create Certified Form 680.” To print and certify a bluecard, open a patient’s record and click the DH680 link. On the DH680 page, click “Create Current Form 680″ after selecting the correct criteria. All valid immunizations required by the bluecard that the patient has received will be inserted. If the proper authorizations have been set, two buttons will appear on the bottom of the screen: “Certify & Print” and “Certify Only.” Before selecting one of these options, the user must select the “Physician or Authorized Signature” from the drop-down box. This name will then appear on the DH680 with an electronic signature which is made up of a unique sequence of letters and numbers. You will then have an electronically-certified DH680 online and ready to use by other providers and schools.
