published by: Stephanie T.Healthcare providers aren’t the only ones who benefit from Florida SHOTS’ electronically-certified DH680. Schools that are enrolled can easily access student immunization records to get a child into the classroom fast.
When your school uses Florida SHOTS, thousands of electronically-certified DH 680s (Blue Cards) are right at your fingertips. We have over 2200 providers currently enrolled and we’re signing up more everyday. When providers electronically-certify 680 forms in Florida SHOTS, they become available for schools to access. After finding a student’s certified form online, simply click the link to view, print, and add to your files. These electronically-certified forms are accepted by the Department of Education when student files are reviewed.
If you find “No Certified 680s on File” when searching a student’s record, this means the student’s immunization provider has not supplied an electronically-signed DH 680. Encourage your parents to ask their immunization provider to electronically-certify their children’s forms in Florida SHOTS.

Evelyn - Schools and preschools can enroll in Florida SHOTS by completing a school application and faxing it to Florida SHOTS. The application is found under the enrollment tab at the top of the page. Just follow the instructions listed there.
Once your account is active, you will be able to view electronically-certified 680 forms and print them out for your files. This does serve as proof of immunization for both the Department of Education and the Department of Children and Families.
