Did you enter shots into a patient record into Florida SHOTS but the 680 is not showing all the shots? Check in your Florida SHOTS database and view the vaccine list for that patient. A red asterisk will appear next to the invalid shot. Place your mouse over the red asterisk for an explanation of the issue regarding that shot entry.

It’s time for Florida teens to call the shots. The Florida Bureau of Immunizations is asking kids and teens ages 11 to 18 to show us their perspective on the two main vaccines affecting their lives, HPV and meningitis, by producing a thirty-second public service announcement (:30 PSA). When it comes to creating certified 680s for direct patient access, you’ve got options.
- You can create both the PIN and the certified 680 at the same time: quickest option for parents and forĀ offices that enter shots given right away since the certified form will be available immediately for direct access.
- You can create a PIN before creating a certified (e-signed) Form 680: handy for providers who want to give the parents an instruction sheet with their PIN but who do not have time to update and create the Form 680 at the time of the visit.
- You can choose not to create a PIN at all for a certified 680: will still give participating schools and daycares access to certified forms but patients will not have direct access to their 680s.

Contrary to what some offices have heard, patient PINs that belong to certified 680s DO NOT EXPIRE.
Once you electronically certify a patient’s D.H. Form 680, the patient PIN remains valid - and the form directly accessible to patients - for as long as the content on the form is valid. The pin will not expire prior to expiration of the certified form.
What expires?
You have 10 days to certify a 680 after creating a PIN. If you don’t certify the 680 in that amount of time, the PIN will be deleted from the system and a new PIN will have to be issued. The certified 680 will continue to be valid as long as the required shots are up-to-date.
