Patients will be able to log onto the secure server at www.flshots.com and print electronically certified copies of D.H. Form 680 from their home computers. No office visit or call is required. August is recognized as National Immunization Awareness Month, an annual event that promotes the benefits and importance of immunizations across the lifespan.
