The Florida SHOTS registry is located on a secure server with password-restricted access. We use the latest encryption and firewall safeguards.
Authorization required. All accounts are given access to the system only after we have verified that they have appropriate licensing in the state of Florida.
Implied consent. Immunization records are confidential medical information—however—health-care providers involved in the care of a patient may share their records with the patients’ other providers. Thus, information like immunization records may be shared between medical professionals without express written consent.
For further information about the Florida statutes covering the sharing of patient records, see Section 456.057, F.S. and Section 440.13(4)(c), F.S.

- Download this flyer to take enrollment and benefit information about Florida SHOTS with you to your next doctor’s appointment.
- Please email your doctor’s information to flshots@doh.state.fl.us , including: doctor’s name, practice or organization name, city, county and telephone number (with area code). We will have a Florida SHOTS regional coordinator follow up with them to make sure they are enrolled in and using the registry.

The electronic signature 680 is now available and accessible to any healthcare provider or school enrolled in Florida SHOTS™. With just a few quick changes to your personnel list, your office can create and certify electronic 680s for your patients.
Benefits of Using Electronically Certified 680s
- Reminder: The doctor’s office creating the form must electronically sign the 680 form in order for it to be accessible for school, daycares and other medical providers.

Florida SHOTS is pleased to announce the development of an online Parent Portal (scheduled for fourth quarter 2010), which will allow health-care providers to give their patients direct, free, online access to their own shot records. Patients will be able to log onto the secure server at www.flshots.com and print electronically certified copies of D.H. Form 680 from their home computers. No office visit or call is required. Start electronically certifying your 680s now in preparation for this exciting tool.

Florida SHOTS helps health-care providers, parents and schools maintain and protect the immunization records that protect Florida’s health
August is recognized as National Immunization Awareness Month, an annual event that promotes the benefits and importance of immunizations across the lifespan.
