- Contact Practice Partner’s Data Interface’s Implementation Specialist Maria Baker at 206-374-4588 to obtain the Florida SHOTS immunization interface.
- Contact Holly Graff, Florida SHOTS Implementation Specialist, at 904-755-5167, and she will help you complete the form to receive a secure ftp site from Florida SHOTS.
- Holly can then schedule an appointment with your office to show a staff member how to easily upload a file.
Immunization data can be uploaded to Florida SHOTS daily, weekly, bi-weekly, or monthly.
